Diversion Coordinator

Reports to

Job purpose
 Under the supervision of the Housing Team Lead, the Diversion Coordinator will be responsible for the coordination and implementation of the Indigenous Diversion program. This position will work with the community to provide alternative resources to homelessness through diversion from shelter, and report on key measurements and performance outcomes.
 
Duties and responsibilities
Develop service plan in collaboration with participants, using the SPDAT and Metis Cart tool
  1. Assist participants in finding housing, provide resource, assist with contact with landlords/property management companies, provide choices or rental options
  2. Connect participants with income supports and assist with the application process
  3. Assist participants to maintain housing
  4. Develop a follow-up visitation schedule based on participants service plan
  5. Facilitate Budgeting 101 and access to RentSmart Basics
  6. Establish effective relationships within Indigenous and non-Indigenous agencies primarily in Central Alberta, serving Indigenous seniors/adults 
  7. Maintain all relevant documentation.
  8. Ensure adherence to all safety protocols
  9. Engaging people and developing trusting relationships.
  10. Administering and analyzing a variety of assessment tools.
  11. Activities, processes, and principles of case management.
  12. Crisis intervention including suicide assessment and prevention
  13. Attend staff meeting, training and board meetings as requested.
  14. Perform additional job-related duties as assigned
 
Skills and Qualifications
Training on dealing with multiple issues and diversity.
  • Minimum of five years working in human services
  • Minimum of five years experience in Indigenous community, equivalencies may be considered
  • Lived experience is an asset
  • Cultural Knowledge regarding issues for Indigenous peoples living off reserve and Metis people
  • Excellent knowledge of communicable diseases, including HIV/AIDS, HCV, TB and COVID 19
  • Excellent knowledge of Indigenous history and the impact of historical violence, intergenerational trauma, residential schools and their affects on substance use, mental health, incarceration, housing, and homelessness on Indigenous peoples
  • Superior communication skills, both oral and written
  • Work-life balance and stress management including burnout avoidance.
  • Self Motivated and Self Directed
  • Able to perform duties with minimal supervision
  • Reliable transportation and ability to travel
  • Social work or related human services diploma degree preferred
  • Specialized education in housing/homelessness, mental health, addictions, and/or Indigenous populations
  • Qualified Indigenous person will be given priority.
 
Working conditions
 The Diversion Coordinator may be exposed to:
  • Stressful situations such as crisis and/or emergencies
  • Job requires sitting for long period of time (during planning and reporting)
  • In accordance with SMLCS health policies, must have appropriate vaccinations for communicable diseases, including Hep A&B, Flu and COVID-19

 20 hours per week -  Monday to Friday, potential for evenings and weekends.

Coordinated Entry worker

Job purpose
 Under the supervision of the Housing Team Lead, the Coordinated Entry staff will be responsible for the coordination and implementation of the Indigenous Coordinated Entry program. This position will work with the community to build safe and trusting environments for Indigenous peoples to engage with the program and report on key measurements and performance outcomes.
 
Duties and responsibilities
Required to use a multi-stage process to pre-screen for eligibility and triage services: 1) initial assessment in HIFIS to determine is currently engaged with another Coordinated Entry team or prevention programming 2) if the participant should be diverted, referred to a prevention program, or referred to the Housing First programs and 3) SPDAT assessment.
  1. Maintain regular contact with the participants while they are on the CAP list.
  2. Connect participants with income supports and assist with the application process.
  3. Required to attend weekly CAP meetings, be able to present participant situations to the CAP committee and make program referral suggestions based on participant need.
  4. Ability to manage a large case load.
  5. Establish effective relationships within Indigenous and non-Indigenous agencies primarily in Central Alberta, serving Indigenous seniors/adults. 
  6. Maintain all relevant documentation.
